Published accounts open with what happened, in the contributor's own framing, with dates the editorial team has checked against a document or a second source.
The middle of a record carries the institutional side: the policy as written, when it took effect, who applied it, and what the institution said at the time. Where the institution declined to comment, that is stated plainly rather than left blank.
Records close with what has and has not changed since. A reversal is noted where one exists. Where a consequence was permanent, the record says so without inflating it.
